If you would like to make an appointment for an iLASIK consultation, you can call us or set up an appointment using the online form below. After you complete the form and submit your information, you will receive a confirmation. A representative from The Eye Center will contact you as soon as possible to schedule your appointment.

If you're making an appointment for the 1st time, please fill out the Patient Registration Form, print it, sign it and bring it with you to your office visit.
